The hardest part of compiling this list, by far, was figuring out the "per 12oz" price at the end of each listing (or per 250g for UK / Australia (coming soon!) listings).
For some vendors and roasters it wasn't too difficult to figure out, but for others, some selling 10oz bags, some selling 1lb bags, some selling 350g bags, etc, plus not including shipping in the quoted price (so I'd have to simulate orders to get estimated shipping costs), it did take some time and some math. I suck at math. ;)
I do think all the quoted "Per 12oz" prices, with shipping costs included in the price, are accurate. However, if anyone notices an error in the high and low prices, please let me know (and demonstrate the math here in forums), and I'll make the corrections.

I think you will find that Has Bean ships internationally (at least to Canada). I just ordered my second 12-week subscription for GBP71 plus GBP50 for shipping to Nova Scotia. Steve's smaller 250g bags flatten nicely in the shipping envelopes.
Update: Roasted December 14, shipped from Stafford UK Dec 17, arrived in my mailbox in Nova Scotia December 21. Image how fast delivery would be if this wasn't the week before Christmas!
