I have been buying coffee from New Harvest roasters in RI for a few years and use their classic espresso (they do not roast it black) for a couple of reasons, taste and the product is organic, shade grown and fair trade. Anyone else have some recommendations for other coffee houses that have excellent organic, fair trade, shade grown coffee. BTW, I also like their price.

You should check out the list of "our favorite roasters" on home barista. There are many great roasters there. With any luck, JasonBrandtLewis will post a hyperlink to it for you.
Unfortunately, I can't think of any that (off the top of my head) definitely meet all of your criteria. However, Red Bird espresso ($51/ 5# shipped!) is (I'm pretty sure) fair trade organic. Klatch roasting has fair trade organic, and they probably carry some that they can attest to being shade grown as well. I'd also check Stumptown and Counter Culture's websites for starters.

Why restrict yourself? Many small growers use organic methods, but the cost of certification is high enough that they can't afford it, so can't market as "Certified Organic". Same with FT certification. Both favor larger growers and leave many microlot folks more or less out in the cold. By insisting on those labels you're actually hurting smaller growers and missing out on a lot of great coffee. Look for roasters who deal directly with individual farmers instead of going by certifications.
