You could try this recipe at INeedCoffee.  If you haven't seen that site, it's worth the browse.

A great smooth add in is Cold Press coffee and you don't need much!!!

Lots of people use powder bases...but stay away from chalking ones...I also recommend even with a powder blend use a 1/4 oz of Vanilla syrup.....rounds it all out.

I worked at a starbucks and when we made the frap base we used all powdered stuff but there were times we had to make it from scratch, All the base is, is:
1/2 espreesso
1/2 milk
some chocalate (sp)
and a very small amount of sugar

and then the diffrent virities had diffrent syrups and chips

the cream base is a little more complicated and everytime I had to make it (with out the powdered base) I screwed it up (basiscly all it is is thick vanilla flavored milk.

At my place, Espresso Euphoria, we have our Polarpower base, which is sugarfree,.. it just helps bind everything together. It is a vanilla bean flavor base so it doesn't taste bitter. Then we have 3 oz cold brew, which is our special coffee blend that is specifically made to be cold, and 3 oz milk. We normally add chocolate, and call it a mochacinno, but we don't have to. Some people prefer them w/o chocolate, with caramel, or any of the other 30 or so flavors we have.
